A paper chart room is two problems at the same time

Hurricane season runs June through November, and paper charts do not survive a roof leak or a flooded storage room. That's the obvious problem. The less obvious one is that damaged charts are still protected information — you don't just lose the record, you inherit an accounting problem for records you can no longer produce. Unlike a server, the chart exists exactly once. Digitizing gives you a second copy, and hands you a recall list on the way out.

Florida dental record retention is set by the Board of Dentistry, and the minimum differs from the general rule that applies to physicians — it also depends on the patient's age at treatment and whether the record is tied to an open matter. We won't quote you a number to plan around. What we can say is that you get complete page-level scans as well as the structured data, so you hold a full record either way. Confirm the current requirement with the Board or your compliance counsel before destroying any original chart.

Both are normal for Miami practices and both are handled. Handwritten intake forms and Spanish-language records process the same way as anything else. Where a field is genuinely illegible or was never filled in — a phone number that changed three times, an email nobody collected in 2004 — we don't guess.
